Dark under-eyes have four distinct causes, and fat repositioning addresses one

A shadow cast by protruding fat, loose skin, pigmentation, and vessels showing through thin skin are separate problems — and some of them are not surgical questions at all.

Last checked:

Lower eyelid fat repositioning moves protruding orbital fat into the hollow below it rather than removing it, easing both the bulge and the hollow at once.

The under-eye is a region where the causes diverge before the treatments do. “My under-eyes look dark” describes several different things, and some of them are not surgical questions.

The four causes

CauseHow it looksDirection of treatment
Orbital fat protrusionThe bulge casts a shadowFat repositioning or removal
Loose skinWrinkling and saggingLower blepharoplasty
PigmentationBrownish discolourationNot a surgical target
Thin skin, visible vesselsBluish castNot a surgical target

More than one frequently coexists. Establishing which applies to you comes before choosing a clinic.

Surgical routes

RouteCharacter
TransconjunctivalNo skin incision
TranscutaneousWhere loose skin is also addressed

Where skin laxity is present, the case moves beyond fat repositioning alone into lower blepharoplasty. Which route fits depends on the degree of fat protrusion and skin condition, and is decided clinically.

Reading how a clinic breaks the options out

Across five Gangnam-area clinics checked on 12 August 2026, the survey also counted whether dark circles were listed as a separate item and whether revision lower blepharoplasty was covered separately. Two of five listed dark circles separately; one listed revision separately.

Separating the items is a clue that a consultation may distinguish between causes — not, in itself, a measure of skill. Since pigmentation and vascular show-through in the table above are not surgical targets, a clinic that reflects that distinction in its material leaves room for a consultation to begin with differential assessment.

What the literature examines

A randomised study compares repositioning orbital fat against grafting it in transconjunctival lower blepharoplasty, and a systematic review covers safety and complications across lower eyelid blepharoplasty generally. Approach selection is a subject of comparative study, not preference. Indication remains a clinical judgement.

Frequently asked questions

What does lower eyelid fat repositioning do?

It addresses both the shadow cast by forward-protruding orbital fat and the hollow of the tear trough beneath it. The aim is to move the fat rather than remove it, evening out the transition.

Are all dark under-eyes candidates for this surgery?

No. A shadow from protruding fat, loose skin, pigmentation, and vessels visible through thin skin are different causes, and some are not treated surgically.

How do the surgical approaches differ?

By route — through the conjunctiva, or through a skin incision. Where loose skin is also present, the case moves into lower blepharoplasty proper.
